HOW TO REPORT A CLAIM

  • Address the immediate needs of the injured worker. If the injury requires immediate attention, call 911.
  • For all non-emergent work injuries, the supervisor and injured worker should call our Society Nurse Triage hotline 24 hours a day, 7 days a week at 877-501-3050 to speak to a nurse regarding the injuries and need for treatment, or to report a claim. A copy of the claim report will be provided to you for your records. Learn more about Nurse Triage 24/7 here.
  • You can also fax your first reports of injury to 920-922-1071 or email claims@societyinsurance.com.

  • What is Society Nurse Triage 24/7?
    Society Insurance has partnered with Medcor to provide our policyholders and their injured workers with direct access to registered nurses following work-related injuries.
  • What will Medcor assist with?
    -A “triage” of the injured workers injury
    -Recommendations for self-care, if appropriate, to avoid unnecessary care for the injured worker and unnecessary expense for the policyholder
    -Recommendations on how and where to get the best possible care at the lowest possible price if treatment is needed for the injured worker’s injuries
    -Submission of the claim to Society Insurance.
